Informed Consent to Treat / Liability Waiver & Release of all Liability In exchange of the fee paid by you to All That Is healing is providing a service using of modalities that may include, but not limited to: Reiki treatment, Intuitive Readings, Crystal and Energy healing, Sound baths, Guided meditations, or Chakra balancing.

I understand that All That Is healing employ complimentary healing modalities that in no way substitutes for medical interventions, body therapy, or psychotherapy.

I acknowledge that an open communication is encouraged between All That Is healing and me to enhance a mutual understanding and acceptance of the energy work provided during my session.

You, the participant, hereby agree to assume all risks associated with any outcome resulting from your participation with All That Is healing programs or group or private consultations.

Side effects may include, but not limited to: Emotional release, lightheadedness, mild fatigue, physical tingling, and change in body temperature which may occur even weeks after the session.

I hold my Healing Practitioner harmless from any possible effects that may cause temporary physical or emotional discomfort and agree to Pamela with All That Is healing to take full responsibility for my self-care and personal development.

I am in control of my own body, and I can always “stop” at any time.

It is my responsibility, as participant, to inform Pamela with All That Is healing if any discomfort should arise.

It is my responsibility, as participant, to notify my practitioner ahead of treatment of any pre-existing medical conditions or injuries and secure clearance to participate.

By signing this Waiver and Release of liability you agree that neither Pamela with All That Is healing shall be responsible for any negative consequence of any kind which is claimed to result from: 1) your participation in any of these programs, activities or consultations; or 2) from the use of any information learned or obtained during these programs, activities or consultations By signing this Waiver and Release you further agree not to assert any legal claims of any kind in any form against Pamela

with All That Is healing on any asserted negative consequence resulting from your participation in these programs, classes or private consultations.

By signing this Waiver and Release you acknowledge and agree that you alone are responsible for your health and wellness and any outcome of the sessions and Pamela with All That IS Healing disclaim all liability for any injuries or negative consequence claimed by you to be the result of your participation in these programs, classes, or private consultations.

I have been encouraged by Pamela with All That Is healing to seek the advice of legal counsel concerning this Acknowledgment and Waiver of Liability; and I execute and deliver this Acknowledgment and Waiver of Liability freely and voluntarily and without duress or coercion and with full Pamela with All That Is healing.

Knowledge of the representations contained herein, and the rights relinquished, surrendered, released and discharged hereunder.

No guarantees can be made for Pamela with All That Is healing sessions and it is recommended that you seek medical treatment and/or evaluation for any medical.

Our sessions are not a substitute for medical treatment or medical evaluation and are not intended for clients who are having a medical crisis.

My signature below also indicates that I am agreeing to the cancellation policy Pamela with All That Is healing that requires a 24-hour cancellation notice or I will be responsible for the full payment of the session.
